I certify that I am the legal owner of the dog(s) I am sending to Pet Care Plus. I have disclosed to Pet Care Plus all known risks, dangers, and medical conditions associated with my dog(s). I understand that Pet Care Plus may refuse service to my dog for any reason it deems necessary, including but not limited to, overly aggressive behavior, overly excessive barking, or illness. I understand that there are many benefits to sending my dog to a dog activity center, and I understand that the benefits outweigh the risks of illness and injury. Such injuries include, but are not limited to cuts, scrapes, punctures, and raw foot pads. Pet Care Plus will not be liable for any illness or injury that may occur to my dog(s). I understand that I am financially responsible for the charges that my pet will incur during his/her stay at Pet Care Plus. All charges will be settled at the time I drop off my dog for his/her stay at Pet Care Plus. I authorize Pet Care Plus to take actions they deem as necessary to ensure the health, well-being, and safety of my dog(s), and to take reasonable action to resolve any medical problems that may arise with my dog(s) us in their care. I agree to assume full financial responsibility for any and all expenses incurred as a result of their actions. I understand that the Pet Care Plus staff is expected to make reasonable attempts to contact me before incurring such expenses, and that an emergency situation could arise where it would not be feasible for them to contact me before the expense is incurred. I hereby release Pet Care Plus Ltd, its agents, officers, sub-contractors, employees, animal owners, customers, and potential customers of Pet Care Plus from any and all liabilities, financial, and otherwise, for injuries to myself, my dog or any other property of mine, which arise in any way from services and/or products provided by or as a consequence of my association with Pet Care Plus. I understand that under no circumstances will Pet Care Plus be liable for damages beyond the replacement value of my dog(s). I certify that, to my knowledge, my dog(s) has not been exposed to bordetella, giardia, distemper, rabies, or parvovirus within the last thirty days. Owner shall hold Pet Care Plus, Ltd. Harmless and indemnify Pet Care Plus, Ltd. From any and all claims arising against Pet Care Plus as the result of any aggressive action of owner's pet resulting in damages or expenses to any other pet upon the Pet Care Plus premises, such indemnification shall extend to any claims or causes of action made by other customers of Pet Care Plus against Pet Care Plus, Ltd., or its employees. Such indemnification is in addition to owner's release of Pet Care Plus, Ltd. From any damages to the owner's pet arising from the aggressive acts of another owner's pet.
